The first line of defence is non-specific (innate): it blocks all microbes alike, without recognising particular germs. The skin is its star: an intact, slightly acidic, dry, keratinised barrier that microbes cannot easily cross, with sweat and sebum adding chemical protection. Skin is categorised as non-specific defence, not acquired immunity.
Mucous membranes line the digestive, respiratory and urinary tracts and trap microbes in sticky mucus. In the respiratory tract, ciliated cells sweep mucus (with trapped germs) upwards to be swallowed or coughed out; sneezing and coughing expel invaders. In the digestive tract, stomach acid (HCl) kills most swallowed microbes, and gut enzymes, bile and the resident gut flora compete with pathogens. Tears (lysozyme), saliva and urine flushing add further barriers.
An antigen is any molecule (usually a foreign protein on a germ's surface) that stimulates an immune response; an antibody is the protein the body makes against it. Antigens trigger immunity; antibodies fight it.
If microbes get past the barriers, the second line (still non-specific) attacks them. Phagocytes engulf and digest microbes: neutrophils are the most abundant, first to arrive, short-lived 'eating' cells; macrophages are larger, longer-lived cells (developed from blood monocytes) that eat germs and dead cells and also present antigens to lymphocytes. Natural killer (NK) cells destroy virus-infected cells and cancer cells without prior sensitisation. Antibodies are not part of non-specific defence — they belong to the specific third line.
Protective proteins: The complement system is a set of plasma proteins that punch holes in bacterial membranes (lysis), attract phagocytes, and coat germs for easier eating. Interferons are proteins released by virus-infected cells that warn neighbouring cells to build antiviral defences, slowing viral spread.
Inflammatory response: Damaged cells release histamines and prostaglandins, which dilate blood vessels and make them leaky — the area becomes red, hot, swollen and painful — while phagocytes flood in to eat germs and clean debris, then healing follows. Temperature response (fever): Pyrogens (fever-causing substances from microbes or the body) act on the hypothalamus, raising the body's set temperature; fever slows many microbes and speeds immune activity.
The third line is specific (acquired) immunity, run by lymphocytes. B lymphocytes (B cells) make antibodies; T lymphocytes (T cells) destroy infected cells. (So: B cells make antibodies and T cells kill infected cells — not the reverse.)
Antibody-mediated (humoral) immunity — B cells: When a B cell meets its matching antigen, it multiplies into plasma cells, which pour out antibodies that neutralise toxins, clump germs (agglutination) and mark them for phagocytes and complement; and into memory B cells, which remember the antigen for a faster, stronger response next time.
Cell-mediated immunity — T cells: Helper T cells recognise antigen (presented by macrophages) and release signals that activate B cells and killer T cells — they coordinate the whole response. Cytotoxic (killer) T cells directly destroy virus-infected cells and cancer cells. Suppressor/regulatory T cells switch the response off afterwards. Cell-mediated immunity works best against virus-infected cells and cancer cells, which antibodies cannot reach inside cells. Monocytes (blood) become macrophages (tissues) and present antigens — they are part of both the second and third lines.
Active immunity means your own body makes antibodies and memory cells: slow to start (days–weeks) but long-lasting. Natural active immunity comes from an infection itself; artificial active immunity comes from vaccination (a weakened/killed germ or its parts, which trains immunity without causing the disease).
Passive immunity means ready-made antibodies are given: immediate protection but short-lived, with no memory. Natural passive immunity passes from mother to baby through the placenta and breast milk. Artificial passive immunity is an injection of antiserum (e.g. anti-tetanus serum, anti-snake-venom serum) for emergencies.
Vaccination works because memory cells make the second response far faster and stronger than the first — the germ is destroyed before it can cause disease. Booster doses refresh memory.
Allergies are over-reactions of the immune system to harmless things (pollen, dust, certain foods). On first exposure the body makes IgE antibodies; on later exposure, the allergen cross-links IgE on mast cells, which release histamine — causing runny nose, sneezing, itchy eyes, skin rashes and swelling. That is why antihistamine medicines relieve allergy symptoms: they block histamine's action. Severe allergy (anaphylaxis) needs emergency adrenaline.
Autoimmune diseases happen when the immune system attacks the body's own tissues, mistaking them for foreign — e.g. rheumatoid arthritis (joints attacked, inflamed and damaged: arthritis inflammation is a misdirected immune response), type 1 diabetes (pancreatic beta cells destroyed), multiple sclerosis (nerve myelin attacked), lupus. Immunodeficiency is the opposite: a weak immune system (e.g. AIDS, where HIV destroys helper T cells) that cannot fight infections.
Transplant rejection: T cells recognise a donor organ's antigens as foreign and attack it. Recipients therefore take immunosuppressant drugs (which suppress immunity) — drugs that strengthen immunity are never prescribed after transplant, because they would worsen rejection. Monoclonal antibodies (identical antibodies from one cloned B-cell line, discovered by Köhler and Milstein) revolutionised research and medicine: they are used in pregnancy tests, cancer therapy, and to identify specific proteins and germs with great precision.

5. What is the difference between B cells and T cells?
Answer: (a) B cells make antibodies, and T cells destroy infected cells — the core division of labour in specific immunity.
6. How do the neutrophils kill microbes?

9. What types of drug are not prescribed to a recipient of organ-transplantation?
Answer: (b) Drugs that strengthen immunity — transplant patients receive immunosuppressants to prevent rejection; strengthening immunity would increase rejection.
10. Cell-mediated immunity works best against;
Answer: (c) Virus infected cells and cancer cells — killer T cells destroy abnormal body cells that antibodies cannot reach inside.
Answer: Helper T cells: recognise antigen and release signals activating B cells and killer T cells (coordination). Cytotoxic/killer T cells: directly destroy virus-infected and cancer cells. Suppressor/regulatory T cells: switch off the immune response after the threat passes. Memory T cells: remember the antigen for faster future responses.
Answer: Skin is the largest barrier: intact keratinised epidermis blocks microbes physically; its dryness and slight acidity discourage growth; sweat and sebum contain antimicrobial chemicals; and its shedding removes attached microbes. A cut in the skin is therefore an open door for infection, which is why wounds must be cleaned and covered.
Answer: Respiratory tract: mucus traps inhaled germs; cilia sweep mucus upwards for coughing/swallowing out; sneezing expels invaders. Digestive tract: stomach acid kills most swallowed microbes; digestive enzymes and bile attack them; normal gut flora out-compete pathogens; gut-associated lymphoid tissue samples antigens and starts immune responses.
Answer: NK cells patrol the body and kill virus-infected cells and tumour cells without needing prior exposure or antibodies: they recognise stressed/abnormal cells (missing normal 'self' markers) and release perforins and granzymes that punch holes in the target and trigger its death. They are an early, non-specific defence against viruses and cancer.
Answer: Pyrogens are fever-producing substances — either microbial products (e.g. bacterial toxins) or body chemicals (e.g. interleukin-1) released during infection. They act on the hypothalamus, raising its temperature set point, so the body produces and conserves heat (shivering, vasoconstriction) until the new, higher temperature (fever) is reached.
Answer: Diseases in which the immune system mistakenly attacks the body's own tissues as if they were foreign. Examples: rheumatoid arthritis (joints), type 1 diabetes (pancreatic beta cells), multiple sclerosis (nerve myelin), systemic lupus. They cause chronic inflammation and tissue damage and are treated with immunosuppressants and symptom control.
Answer: (i) Antigen triggers immunity (foreign molecule); antibody is the protein made against it. (ii) B cells make antibodies; T cells kill infected cells/coordinate. (iii) Macrophages: large, long-lived, present antigens; neutrophils: small, abundant, first-arriving, short-lived phagocytes. (iv) Complement: plasma proteins that lyse bacteria and attract phagocytes; interferons: antiviral proteins warning neighbouring cells. (v) Innate: present from birth, non-specific, no memory; acquired: specific, with memory, develops after exposure. (vi) Active: body makes its own antibodies (slow, lasting); passive: ready-made antibodies given (immediate, temporary). (vii) Natural active: after infection; artificial active: after vaccination. (viii) Cell-mediated: T cells destroy abnormal cells directly; antibody-mediated: B-cell antibodies fight germs in fluids.
Answer: Neutrophils are the first phagocytes at an infection: they squeeze out of blood vessels, engulf bacteria into a phagosome, fuse it with lysosomes, and digest the bacteria with enzymes and reactive oxygen chemicals; then they die (forming pus). Macrophages arrive later and live longer: they engulf bacteria and debris, digest them, and present bacterial antigens on their surface to helper T cells, linking non-specific defence to specific immunity. Together they clear most bacterial invasions before antibodies are even needed.
Answer: Complement proteins activate in a cascade on bacterial surfaces and assemble membrane attack complexes (MAC) — pores punched through the bacterial membrane — so water rushes in and the bacterium bursts (lysis). Complement fragments also coat bacteria (opsonisation) for easier phagocytosis and attract phagocytes to the site. Interferons, released by virus-infected cells, bind receptors on neighbouring healthy cells and switch on antiviral genes: those cells make enzymes that degrade viral RNA and block viral protein synthesis, so even if the virus enters, it cannot replicate well.
Answer: Inflammation is generalised because it is triggered by any tissue damage or infection — not by a specific germ — and follows the same pattern everywhere: damaged cells and mast cells release histamine and prostaglandins; vessels dilate and leak; blood flow, phagocytes and plasma proteins flood the area; germs and debris are cleared; then repair begins. It works against bacteria, viruses, fungi and injuries alike, needs no prior exposure and has no memory — the definition of a non-specific defence.
Answer: Monocytes are blood white cells that enter tissues and become macrophages — phagocytes that eat germs and present antigens, bridging innate and acquired immunity. T cells (matured in the thymus) run cell-mediated immunity: helpers coordinate, killers destroy infected/cancer cells, regulators switch off responses. B cells (matured in bone marrow) run antibody-mediated immunity: on meeting their antigen they become plasma cells making antibodies and memory cells for future protection. Together, phagocytes present, T cells direct and kill, B cells arm the fluids with antibodies.
Answer: Active immunity: the person's own immune system responds — B and T cells activate, antibodies are made, memory cells form. It takes days to weeks to develop, but protection lasts months to years (often life). Examples: recovery from infection, vaccination. Passive immunity: ready-made antibodies are transferred — no immune response or memory in the recipient. Protection is immediate but lasts only weeks to months. Examples: maternal antibodies through placenta/milk, antiserum injections.
Answer: Helper T cells (CD4): after macrophages present antigen, helpers release cytokines that activate B cells (antibody production) and cytotoxic T cells, and recruit macrophages — they are the coordinators; their loss in AIDS cripples immunity. Cytotoxic T cells (CD8): recognise antigen on infected/cancer cells and kill them by releasing perforin and granzymes. Memory T cells persist after infection for rapid re-attack. Suppressor/regulatory T cells dampen the response to prevent damage to healthy tissue.
Answer: Each B cell carries surface antibodies for one specific antigen. On meeting that antigen (often with helper-T-cell help), the B cell multiplies: most descendants become plasma cells, which secrete large amounts of matching antibody into blood and lymph. Antibodies neutralise toxins and viruses, agglutinate germs, activate complement, and mark germs for phagocytosis. Other descendants become memory B cells, giving faster, stronger secondary responses on re-exposure — the basis of lasting protection after infection or vaccination.
Answer: An allergy is an exaggerated immune response to a normally harmless substance (allergen) such as pollen, dust-mite droppings or peanut protein. First exposure sensitises: IgE antibodies are made and attach to mast cells. On re-exposure, the allergen binds this IgE, mast cells release histamine (and other mediators): histamine dilates vessels and increases leakiness (swelling, redness), stimulates mucus glands (runny nose), irritates nerves (itching, sneezing) and constricts airways (wheezing in asthma). Antihistamines block histamine receptors, easing these symptoms.
Answer: A transplanted organ carries donor antigens (especially MHC/HLA molecules) that the recipient's T cells recognise as foreign. Helper T cells activate cytotoxic T cells, which directly attack and kill the graft's cells, and activate B cells, which make antibodies against the graft — together causing acute rejection. Matching donor and recipient HLA as closely as possible reduces this, and lifelong immunosuppressant drugs (which dampen T-cell activation) are needed to keep the graft alive. Chronic rejection is slower immune damage to the graft's vessels over years.
Answer: In rheumatoid arthritis, the immune system attacks the joint lining (synovium) as if it were foreign. The result is chronic inflammation in the joint: histamine and prostaglandins dilate vessels, fluid and immune cells flood in, and the swollen, painful, stiff joint is damaged over time by the very cells meant to protect the body. Inflammation here is misdirected — its target is the body's own tissue, not a germ — so anti-inflammatory and immunosuppressant drugs help by calming the response.
Answer: Fever does help fight infection, but a high or prolonged fever harms the patient: it causes dehydration, exhaustion, delirium, fits in children (febrile seizures), and in extreme cases organ damage. Antipyretics (e.g. paracetamol) lower the hypothalamic set point to a safer level — they do not stop the immune response, they only prevent fever itself from becoming dangerous, while the body keeps fighting the infection by other defences. Doctors balance the benefit of fever against its risks, especially in children, the elderly and the very ill.
Answer: Runny nose and many skin rashes in allergy are caused mainly by histamine released from mast cells: histamine increases vessel leakiness and mucus secretion (runny nose), and irritates skin nerves (itching, red welts). Antihistamines block histamine receptors (H1), so even though histamine is released, it cannot act — symptoms ease quickly. They treat the mechanism of the symptoms, not the allergy itself, which is why avoidance of the allergen and, in some cases, desensitisation are also used.
Answer: In 1975, Köhler and Milstein fused a mouse B cell (making one specific antibody) with a myeloma (cancer) cell, creating a hybridoma — an immortal cell line producing unlimited amounts of one identical (monoclonal) antibody. This revolutionised research and medicine: for the first time, scientists could obtain pure, specific antibodies on demand — for identifying proteins and cell types, pregnancy and disease tests, purifying substances, and targeted cancer therapy (antibodies carrying drugs to tumour cells). It won the 1984 Nobel Prize and founded the modern antibody industry.
