ENGLISH

Medical physics applies physical principles and techniques to medicine — especially to the diagnosis and treatment of disease. Its job, in one sentence, is to help doctors see inside the human body and treat patients safely: improving imaging accuracy, reducing radiation risks, and improving treatment. The examples you already know from everyday life are its products: X-ray imaging, ultrasound scans, CT scans and PET scans, used to detect fractures, diagnose tumours and study internal organs — all without surgery.

The working material of the whole chapter is radiation: the transfer of energy through space or matter as waves or particles. In medical physics radiation is used for both diagnosis (imaging) and treatment, and the first classification you must master splits it into two classes. Ionizing radiations have enough energy to knock electrons out of atoms, producing ions — X-rays and gamma rays belong here, and that ionising power is exactly why they can damage living tissue and why their doses are carefully controlled. Non-ionizing radiations do not have enough energy to ionize atoms — ultrasound, visible light and radio waves belong here, and that is why ultrasound can be repeated safely, even in pregnancy.

How do the ionizing radiations interact with matter? Mainly through two processes you already met in Chapter 18: the photoelectric effect (a photon is absorbed and ejects an electron) and Compton scattering (a photon bounces off an electron and loses energy). Those two interactions are what make X-ray images possible — tissues absorb differently, so the beam that comes out carries a shadow picture of what it passed through — and they are also what makes radiation dangerous in large doses, since every ionization is a small chemical disruption in a cell. The positrons used in PET scanning come from radioactive isotopes that decay by positron emission, which is why PET belongs to nuclear medicine rather than to X-ray imaging. The rest of this chapter covers the four imaging techniques in turn: PET, ultrasound, X-rays and CT — each with its own physics, its own strengths, and its own limits.

Non-ionizing Ultrasound, visible light, radio waves Ionizing X-rays, gamma rays (remove electrons) Ionizing radiation can ionize atoms; non-ionizing cannot

ENGLISH

PET (Positron Emission Tomography) works unlike any other scan in this chapter: it does not photograph anatomy, it maps activity. The steps are worth learning in order. First, a radioactive tracer — typically glucose labelled with fluorine-18 — is injected into the patient. It travels in the blood and concentrates where metabolic activity is high, because active cells burn more glucose. Malignant (cancer) tissues are famously greedy for glucose, so they light up; so do active regions of the brain. Second, the tracer decays, emitting positrons. Each positron travels only a millimetre or so before it meets an electron in the tissue, and they annihilate: e+ + e− → 2 gamma-ray photons, each of 0.511 MeV, flying in exactly opposite directions to conserve momentum. The radiation detected in a PET scan is therefore gamma rays (the book's MCQ 20.1) — not positrons, which never leave the body.

The cleverness is in the detection. The scanner is a ring of detectors around the patient. When two detectors on opposite sides record gamma rays simultaneously (within nanoseconds), the computer knows the annihilation happened somewhere on the line joining them — this is coincidence detection (the book's MCQ 20.2). Millions of such lines, collected from millions of annihilations, build up a 3D map of tracer concentration: bright spots show high metabolic activity — tumours, active brain regions, a healing wound burning energy. A further refinement, time-of-flight measurement, uses the tiny time difference between the two detections to narrow down where along the line the event occurred (the book's constructed-response question 20.1), sharpening the map still further. And lead collimators in front of the detectors block gamma rays arriving from wrong directions, sharpening the image (question 20.6) — lead stops stray gammas the way blinkers stop side vision.

Why does PET matter when CT already shows anatomy so well? Because disease often changes function before it changes shape. A tumour may be metabolically abnormal while it is still too small or too similar to surrounding tissue to stand out on a CT image — PET catches the abnormal glucose appetite first. The price is the short-lived tracer: fluorine-18 has a half-life of about 110 minutes, so the tracer must be made nearby (often in a hospital cyclotron) and used quickly. PET's other limit is resolution: it maps activity in centimetre-scale regions, not the millimetre anatomy of CT — which is why hospitals often combine the two, PET for “where is the disease active” and CT for “exactly what does it look like”.

Annihilation: e⁺ + e⁻ → 2 gamma photons (0.511 MeV each, opposite directions)
Ultrasound and the Piezoelectric Effect

ENGLISH

Ultrasound is a widely used non-invasive imaging technique that uses sound waves above human hearing (> 20,000 Hz) to visualise internal structures — tissues, organs, and the fetus during pregnancy. The waves used are longitudinal sound waves (the book's MCQ 20.7): compressions and rarefactions travelling through tissue, like ordinary sound but far too high-pitched to hear. Two consequences follow at once. Ultrasound is not electromagnetic radiation and not ionizing radiation, so it is considered safe and can be repeated as often as needed — the reason it is the standard choice in pregnancy. And because it is sound, it needs a medium: it travels through tissue and fluid, but not through vacuum, and barely through air — a limitation the next section turns into a diagnostic rule.

The device that makes ultrasound possible is the transducer, and its heart is the piezoelectric effect. Certain crystals — quartz and PZT (lead zirconate titanate) are the book's examples — show a two-way effect: squeeze the crystal and it develops a voltage; apply a voltage and the crystal changes shape (vibrates). That two-way street lets one transducer do both jobs (the book's MCQ 20.3 and short question 20.2). An alternating voltage makes the crystal vibrate, emitting ultrasound pulses into the body. Then the transducer switches to listening: returning echoes squeeze the crystal, generating a voltage signal that is detected and measured. Send a short pulse, wait for echoes, measure their return times — the machine builds the image from those times, exactly as sonar maps the seabed and radar maps the sky, but with sound inside the body.

The pulse-and-listen rhythm matters for the numbers. Sound travels through soft tissue at roughly 1,540 m s−1, so an echo returning after 100 microseconds has come from a boundary about 7.7 cm deep (half the round trip). Higher frequencies give finer detail but penetrate less deeply, because tissue absorbs them faster — so the sonographer chooses the frequency for the job: high frequency for a shallow, detailed view (like the eye), lower frequency for a deep view (like the abdomen). The transducer's crystal frequency is therefore not a detail: it sets the trade between seeing fine and seeing deep, and every ultrasound machine offers several probes for exactly that reason.

Piezoelectric crystal Voltage → vibration (emit) • Vibration → voltage (detect) One transducer both sends and receives ultrasound

How does a stream of echoes become a picture? (the book's comprehensive question 20.2). When ultrasound hits the boundary between two tissues, part of it is reflected back as an echo (the book's short question 20.5) and part continues deeper. The machine records each echo's return time — which gives the depth of the boundary, since sound speed in tissue is known — and its strength — which tells how different the two tissues are acoustically. From thousands of echoes, fired in a fan of directions, it builds a cross-sectional image: bright lines where strong echoes came from sharp boundaries, grey shades where echoes were weak, black where nothing came back. That is the ultrasound image on the screen: a slice through the body, drawn in echoes.

The parts commonly examined (the book's MCQ 20.4 and short question 20.3) show how general the tool is: the abdomen (liver, kidneys, gallbladder), pregnancy (the fetus, its growth and heartbeat), muscles and soft tissues, the heart (echocardiography — the beating heart, valve by valve, in real time), and blood flow (Doppler ultrasound, which uses the frequency shift of echoes from moving blood to measure flow speed and direction — the same Doppler effect as a passing ambulance siren, applied to red blood cells).

The great limitation is air (the book's constructed-response question 20.2): ultrasound is not effective for lungs or gas-filled intestines. Sound is almost completely reflected at a tissue–air boundary — a huge acoustic impedance mismatch — and barely enters the air-filled organ, so almost no echoes return from inside it. The screen beyond such a boundary goes dark, not because there is nothing there but because the sound never got in. The same rule explains two practical details: the sonographer spreads coupling gel on the skin (to remove the air layer between probe and skin, which would reflect the pulse before it even entered the body), and a full bladder is sometimes requested for pelvic scans (fluid gives sound a window where gas would block it). Know the rule and the exceptions make sense: ultrasound sees fluid and soft tissue beautifully, and air hardly at all.

X-rays — Production, Types and Contrast
🏷 Tags: X-rays, bremsstrahlung, characteristic X-rays, K-alpha, K-beta, contrast medium
ENGLISH

Production: X-rays are produced by electron bombardment of a metal target. Fast electrons from a heated cathode are accelerated across a high voltage onto a tungsten target, and two processes occur at once. (1) Bremsstrahlung (“braking radiation”): electrons decelerated by the electric fields of target nuclei emit a continuous spectrum of X-rays — a smooth range of all wavelengths down to a minimum set by the electron energy, λmin = hc/eV. Raise the tube voltage and the minimum wavelength shrinks: harder, more penetrating X-rays. (2) Characteristic X-rays: an incoming electron knocks out an inner-shell electron, leaving a vacancy; an outer electron drops in to fill it, emitting a photon of sharply defined energy fixed by the target element — sharp lines on top of the smooth bremsstrahlung background (the book's MCQ 20.8).

The characteristic lines have names you must know (the book's short question 20.7). When a K-shell (innermost) vacancy is filled by an L-shell electron, the emitted photon is Kα. When the same K vacancy is filled by an M-shell electron, the photon is Kβ — higher energy and shorter wavelength, since the M shell is farther out and the electron falls further. Tungsten's K lines sit around 60 keV, which is why tungsten targets are standard: their characteristic radiation is in the useful diagnostic range, and tungsten's high melting point survives the bombardment heat.

Contrast is what makes the image readable (the book's short question 20.4 and MCQ 20.5): contrast in an X-ray image mainly depends on the density and thickness of the tissues. Dense, thick tissue (bone) absorbs more X-rays and appears white; soft tissue absorbs less and appears dark — the film or detector records what got through, so the image is a negative of absorption. The low-contrast problem (the book's constructed-response question 20.3) arises when two tissues absorb almost equally: they look the same and the boundary vanishes. The solution is a contrast medium: a substance that absorbs X-rays strongly, given to the patient to outline the structure — a barium meal for the gut, iodine dye for blood vessels and kidneys. The medium paints the invisible visible, turning two identical greys into a sharp black-and-white edge. It is the same idea as dye in a plumbing test: make the thing you want to see absorb differently from everything around it.

Target Bone (white): absorbs more Soft tissue (dark): absorbs less Contrast = difference in X-ray absorption (density × thickness)

CT stands for Computed Tomography (the book's MCQ 20.9). It combines X-ray technology with computer processing to generate high-resolution images — and the combination is more than the sum of its parts. How it works: an X-ray tube rotates around the patient, taking many X-ray images of the same section from different angles. A computer combines these into a 2D image of that section — a slice. The process repeats along the body axis, and the 2D slices are combined into a 3D image of the internal structure. The patient lying in the doughnut, the tube whirring around them, is the most recognisable image in modern medicine — and the physics behind it is pure Chapter 20: X-ray absorption, measured from every angle, reconstructed by mathematics.

The advantage over a plain X-ray (the book's MCQ 20.6, short question 20.6 and comprehensive question 20.4) is the removal of overlap. A plain X-ray squashes all structures into one flat image, so organs overlap and hide each other: a rib can hide a lung lesion, the spine can hide a kidney stone. CT separates them slice by slice, so each slice shows only the structures in that thin section — far better clarity and diagnostic value for tumours, internal bleeding and fractures hidden behind other structures. The book's constructed-response question 20.4 states the same point from the other side: because each CT slice is thin and viewed separately, structures in front and behind no longer overlap in the image. Overlap was the plain X-ray's fundamental weakness, and CT was invented to remove it.

There is a trade-off, and the book is honest about it: a CT scan uses more radiation than a single X-ray, because it takes many X-ray images to build one slice. It is therefore used only when the extra detail is needed — a head injury where bleeding must be found fast, a tumour whose exact size and position must be mapped before surgery, a fracture invisible on a plain film. That cost–benefit judgement — more dose, much more information — is itself medical physics: the physicist's job is not only to build the machine, but to make sure each scan is justified, the dose is as low as reasonably achievable, and the image answers the doctor's question. CT is the chapter's closing example of the whole subject's promise: physics, turned into a picture, turned into a diagnosis.

Patient sliding into a CT scanner doughnut

Short Answer Questions

20.1 1. Which type of radiation is produced during a PET scan?

Gamma radiation — the two 0.511 MeV gamma-ray photons produced by each positron–electron annihilation.

20.2 2. Which physical effect enables ultrasound transducers to produce sound waves?

The piezoelectric effect — applying an alternating voltage to a piezoelectric crystal makes it vibrate, emitting ultrasound.

20.3 3. Mention one area of the body commonly examined using ultrasound.

The abdomen (liver, kidneys, gallbladder) — or pregnancy (fetus), or the heart (echocardiography).

20.4 4. What causes contrast in X-ray images of different body tissues?

Different absorption of X-rays: denser and thicker tissues (bone) absorb more X-rays and appear white, while softer tissues absorb less and appear darker. Contrast depends mainly on tissue density and thickness.

20.5 5. What occurs when ultrasound hits the boundary between two tissues?

Part of the ultrasound is reflected back as an echo and part is transmitted into the next tissue. The reflected echo is detected by the transducer and used to build the image; the echo strength depends on how different the two tissues are.

20.6 6. How does a CT scan improve imaging? Explain briefly.

A CT scan takes many X-ray views of one thin section from different angles and reconstructs a 2D slice image, then stacks slices into 3D. This removes the overlap of organs seen in a plain X-ray, giving much clearer images and better diagnosis.

20.7 7. What do you know about the terms Kα and Kβ X-rays?

Kα and Kβ are characteristic X-rays from K-shell vacancies: Kα is emitted when an L-shell electron fills a K-shell vacancy; Kβ when an M-shell electron fills it. Kβ has higher energy (shorter wavelength) because the M shell is farther from the nucleus than the L shell.

Constructed Response Questions

20.1 1. What does a time difference in detecting gamma rays at two detectors indicate in a PET scan?

In time-of-flight PET, the two annihilation photons reach opposite detectors at slightly different times if the annihilation did not happen exactly midway. The time difference tells how far along the line between the detectors the event occurred — the photon arriving first came from a point closer to that detector. This narrows the event location and improves image quality.

20.2 2. Why is ultrasound not effective for imaging air-filled organs like the lungs? Explain.

Because of the huge acoustic impedance mismatch at a tissue–air boundary: almost all the ultrasound is reflected and almost none is transmitted into the air-filled organ. With no sound entering the lungs, no echoes return from inside them, so no image of their interior can be formed. A coupling gel is used on the skin for the same reason — to remove air gaps.

20.3 3. What happens when the X-ray contrast between two tissues is too low? Suggest a solution.

The two tissues look almost identical on the image and cannot be distinguished — structures are missed. Solution: use a contrast medium — a substance that absorbs X-rays strongly (barium sulphate meal for the digestive tract, iodine-based dye for blood vessels and kidneys) — which outlines the structure against surrounding tissue.

20.4 4. How does Computed Tomography scanning reduce the overlapping of organs seen in plain X-rays? Explain.

A plain X-ray compresses the whole body thickness into one flat image, so organs in front and behind overlap. CT images only one thin section at a time: X-rays from many angles are combined into a 2D slice, and each slice is viewed separately. Structures outside that slice do not appear in it, so overlap is removed and hidden details become visible.

20.5 5. What could be a possible medical cause of uneven tracer concentration seen in a PET brain scan?

A tumour (high metabolic activity concentrates more tracer), a stroke (damaged tissue takes up less tracer), or abnormal brain activity such as epilepsy or Alzheimer's disease (regions of reduced or increased metabolism). Uneven uptake marks regions where metabolism differs from normal tissue.

20.6 6. What is the purpose of lead collimators in PET gamma-ray detectors? Explain.

Lead collimators are lead shields with narrow openings placed in front of the detectors. They absorb gamma rays arriving from directions other than the detector's line of view, so only photons travelling along the expected path are counted. This reduces background and scattered radiation, sharpening the image and improving the accuracy of coincidence detection.

Comprehensive Questions

20.1 1. What is the piezoelectric effect, and how does it allow transducers to both emit and detect ultrasound waves?

The piezoelectric effect is the generation of electric charge by certain crystals when mechanically stressed, and the reverse — mechanical deformation when an electric field is applied. In a transducer, an alternating voltage makes the crystal vibrate at ultrasonic frequency, emitting ultrasound pulses. When returning echoes strike the crystal, they squeeze it, generating a voltage signal. The same crystal therefore works as both loudspeaker and microphone for ultrasound, switching rapidly between sending and listening.

20.2 2. Describe how an ultrasound image is created using the concepts of echoes and reflected sound waves.

The transducer emits a short ultrasound pulse into the body. At each boundary between tissues of different acoustic impedance, part of the pulse reflects as an echo. The transducer detects the echo and the machine records its return time (giving the boundary's depth: depth = speed × time/2) and its strength (giving how different the tissues are). By sweeping the beam and combining thousands of echoes, the machine builds a two-dimensional cross-sectional image in real time.

20.3 3. Explain how attenuation of X-rays by different tissues creates contrast in the resulting image. Include an example.

As X-rays pass through the body they are attenuated (absorbed/scattered) more by dense, thick, high-atomic-number tissue and less by soft, thin tissue. The detector (or film) behind the body therefore receives different intensities: regions behind bone receive few X-rays (appear white), regions behind soft tissue receive many (appear dark). Example: a chest X-ray shows white ribs and spine against dark lungs, because bones attenuate strongly while air-filled lungs attenuate weakly.

20.4 4. Compare CT and conventional X-ray imaging with respect to image clarity and diagnostic value.

Conventional X-ray: one flat projection — fast, cheap, low dose, good for fractures and chest screening, but organs overlap and soft-tissue detail is poor. CT: many projections reconstructed into thin slices and 3D — far better clarity, soft-tissue contrast and diagnostic value (tumours, bleeding, complex fractures, treatment planning), but higher radiation dose, cost and time. CT is chosen when plain X-ray detail is insufficient.

20.5 5. Write a brief note on PET. How is it used to scan malignant tissues?

PET (Positron Emission Tomography) is a nuclear-medicine imaging technique. A tracer — usually glucose labelled with fluorine-18 — is injected; it accumulates in metabolically active cells. The tracer emits positrons, which annihilate with electrons, producing pairs of 0.511 MeV gamma photons in opposite directions. A ring of detectors records coincident pairs, and a computer reconstructs a 3D map of tracer uptake. Malignant tissues grow fast and consume much more glucose than normal tissue, so they appear as bright "hot spots" — allowing detection, staging and treatment monitoring of cancers.

20.6 6. How would you explain the inner shell transitions of X-rays? Also explain characteristic and continuous X-rays.

Inner-shell transitions: a fast electron knocks an electron out of an inner shell (K or L) of a target atom, leaving a vacancy. An outer-shell electron drops into the vacancy, and the energy difference is emitted as an X-ray photon with an energy fixed by the element — these are characteristic X-rays (Kα, Kβ, …), appearing as sharp lines. Continuous X-rays (bremsstrahlung, "braking radiation") come from a different process: incident electrons decelerated by nuclei emit photons of all energies up to a maximum (set by the tube voltage), forming a smooth continuous spectrum. A real X-ray tube spectrum shows both: a continuous background with sharp characteristic peaks on top.

ENGLISH

Medical physics turns physics into pictures that save lives, and this chapter teaches four of them. It starts with the split that governs every choice: ionizing radiation (X-rays, gamma rays — they knock electrons out of atoms) versus non-ionizing radiation (ultrasound, light, radio waves — they do not), with X-rays and gammas interacting mainly by the photoelectric effect and Compton scattering of Chapter 18. PET maps activity, not anatomy: a fluorine-18 glucose tracer concentrates where cells burn energy, its positrons annihilate into pairs of 0.511 MeV gamma rays flying in opposite directions, and a ring of detectors reconstructs the activity map by coincidence detection — tumours betray themselves by their appetite. Ultrasound maps echoes: a piezoelectric crystal (quartz, PZT) both sends sound above 20,000 Hz and listens for its return, and return time gives depth, strength gives tissue difference — safe, repeatable, ideal in pregnancy, but blind where air blocks sound. X-rays map absorption: bremsstrahlung plus characteristic lines (Kα, Kβ) from a tungsten target, contrast set by density and thickness (bone white, soft tissue dark), low contrast rescued by barium or iodine media. And CT maps slices: a rotating tube and a computer turn many angled X-ray views into thin sections and then a 3D volume, removing the overlap that hides disease on a plain film — at the price of a higher dose, used only when the detail is worth it.

Topics in this chapter: Medical Physics and Radiations, PET Scanning, Ultrasound and Piezoelectric Effect, Diagnostic Ultrasound, X-rays, CT Scanning, Official Exercise (28 questions).
