ENGLISH

The human urinary system has: two kidneys, two ureters, one urinary bladder and one urethra. The kidneys filter blood; ureters carry urine to the bladder; the bladder stores it; the urethra passes it out.

Each kidney is bean-shaped, about the size of a fist, lying against the back wall of the abdomen. Cut open, it shows an outer cortex, an inner medulla (made of cone-like renal pyramids) and a central hollow renal pelvis that collects urine into the ureter.

Blood enters through the renal artery and leaves through the renal vein. The kidney does two jobs at once: excretion (removing urea, salts, excess water) and osmoregulation (keeping water and salt balance constant) — that is why the book calls it both an excretory and an osmoregulatory organ.

ENGLISH

Each kidney contains about one million nephrons — the functional units of the kidney. A nephron has two main parts: the renal corpuscle and the renal tubule.

The renal corpuscle = glomerulus (a ball of capillaries) + Bowman's capsule (a cup around it). Blood enters the glomerulus by the afferent arteriole (wider) and leaves by the efferent arteriole (narrower) — the narrow exit raises pressure, which drives filtration.

The tubule runs: Bowman's capsule → proximal convoluted tubule (PCT) → loop of Henle (down into the medulla and back) → distal convoluted tubule (DCT) → collecting duct (shared by many nephrons — so the collecting duct is not part of a single nephron). Cortical nephrons (≈85%) stay mostly in the cortex; juxtamedullary nephrons have long loops reaching deep into the medulla and are key for concentrating urine.

3. Urine Formation — Three Processes

ENGLISH

(i) Glomerular filtration: High pressure forces water, salts, glucose, amino acids and urea out of glomerular blood into Bowman's capsule. Blood cells and large plasma proteins are too big to pass, so they stay in blood — that is why the fluid in Bowman's capsule contains only small molecules.

(ii) Selective reabsorption: As filtrate flows through the tubule, useful substances are taken back into the blood by peritubular capillaries — all glucose is reabsorbed (in the PCT, using energy from mitochondria), plus most water, salts and amino acids. That is why a healthy person's urine has no glucose.

(iii) Tubular secretion: Extra wastes (hydrogen ions, potassium, some drugs, creatinine) are secreted from blood into the tubule. What remains is urine, which flows through the collecting duct to the renal pelvis.

ENGLISH

Counter-current mechanism: The loop of Henle and the collecting duct run in opposite directions, building a salt gradient in the medulla — saltier towards the bottom. Water leaves the collecting duct by osmosis into this salty medulla, so urine becomes concentrated. Without loops of Henle, only dilute urine could form.

ADH (antidiuretic hormone): Made by the hypothalamus and released from the posterior pituitary when blood is short of water. ADH makes the collecting duct walls more permeable to water, so more water is reabsorbed, blood volume rises and urine is small and concentrated. When water is plentiful, ADH falls, the duct becomes less permeable, and urine is large and dilute. A person who cannot make enough ADH (pituitary damage) passes huge volumes of dilute urine constantly.

Aldosterone (from the adrenal cortex) regulates sodium and potassium: when blood sodium is low, aldosterone increases sodium reabsorption (and water follows), raising blood volume and pressure.

ENGLISH

Urinary tract infections (UTIs) are bacterial infections of the urinary tract, more common in women because of a shorter urethra. Kidney stones are mostly calcium oxalate (also uric acid stones when uric acid is high). Small stones may pass naturally; larger ones are broken by lithotripsy — extracorporeal shockwave lithotripsy (ESWL, shockwaves from outside) or percutaneous nephrolithotripsy (PNL, through a small skin incision).

Kidney failure may be acute (sudden, often reversible — e.g. injury, severe infection, blocked urine flow) or chronic (slow, irreversible damage — e.g. diabetes, high blood pressure). When kidneys fail, wastes build up and dialysis or transplant is needed.

Dialysis: In haemodialysis, blood is pumped through a dialyser machine where wastes diffuse into dialysate flowing in the opposite direction (counter-current keeps the gradient steep); cleaned blood returns to the body. In peritoneal dialysis, the abdomen lining (peritoneum) acts as the filter. Kidney transplant replaces the failed kidney with a donor kidney; recipients must take immunosuppressant drugs for life to prevent rejection, which raises infection risk and can cause side effects like high blood pressure and weight gain.

Section 2: Short Questions
SHORT + ANSWERS
1. Justify the functioning of kidneys as both excretion and osmoregulation.

Kidneys excrete nitrogenous wastes (urea), excess salts and drugs — that is excretion. At the same time they adjust how much water and salt leave in urine (via ADH, aldosterone and the counter-current mechanism), keeping blood volume and salt concentration constant — that is osmoregulation.

2. List the organs of the urinary system.

Two kidneys, two ureters, one urinary bladder and one urethra.

3. Define glomerular filtration.

The filtration of blood plasma under high pressure through the glomerular capillaries into Bowman's capsule. Water, salts, glucose, amino acids and urea pass through; blood cells and plasma proteins do not.

4. Compare the function of glomerular capillaries and peritubular capillaries.

Glomerular capillaries filter blood under high pressure into Bowman's capsule (filtration out). Peritubular capillaries surround the tubule and take reabsorbed substances back into blood, and give up extra wastes for secretion (reabsorption and secretion).

5. What are the three main processes involved with urine formation?

Glomerular filtration, selective reabsorption and tubular secretion.

6. What are the causes of kidney failure?

Acute causes: sudden injury, severe infection, blocked urine flow, very low blood pressure. Chronic causes: diabetes, high blood pressure, long-term kidney inflammation or repeated infections, and some inherited disorders.

7. Why do blood and dialysate flow in opposite direction?

Opposite (counter-current) flow keeps a steep concentration gradient along the whole dialyser, so wastes keep diffusing out of blood into dialysate efficiently instead of reaching equilibrium early.

8. Differentiate between: (i) Afferent and efferent arterioles (ii) Chronic and acute renal failure (iii) Haemodialysis and peritoneal dialysis (iv) Renal cortex and renal medulla.

(i) Afferent arteriole brings blood into the glomerulus and is wider; efferent carries blood away and is narrower, raising filtration pressure. (ii) Acute failure is sudden and often reversible; chronic failure is slow, progressive and irreversible. (iii) Haemodialysis cleans blood in an external machine; peritoneal dialysis uses the abdomen lining as filter. (iv) Cortex is the outer region containing glomeruli and convoluted tubules; medulla is the inner region with loops of Henle and collecting ducts in renal pyramids.

Section 3: Long Questions
LONG + ANSWERS
1. Describe the structure of kidney and relate it with its function.

The kidney is bean-shaped with an outer cortex (glomeruli, PCT, DCT), an inner medulla of renal pyramids (loops of Henle, collecting ducts) and a renal pelvis draining into the ureter. The cortex filters blood; the medulla's salt gradient concentrates urine; the pelvis collects and carries urine out. The renal artery brings blood to be cleaned and the renal vein returns cleaned blood — structure matches the jobs of filtration, reabsorption and concentration.

2. Explain the detailed structure of nephron.

A nephron = renal corpuscle (glomerulus + Bowman's capsule) + renal tubule (PCT → loop of Henle → DCT → collecting duct, shared). Afferent arteriole (wide) enters, efferent (narrow) leaves, raising pressure for filtration. Peritubular capillaries surround the tubule for reabsorption/secretion. Cortical nephrons (85%) are short-looped; juxtamedullary nephrons have long loops deep in the medulla with vasa recta, and concentrate urine.

3. Explain the main processes in kidney functioning.

Filtration: pressure forces small molecules from glomerular blood into Bowman's capsule. Selective reabsorption: PCT and other tubule parts return all glucose, most water, salts and amino acids to blood via peritubular capillaries (needs ATP). Tubular secretion: H+, K+, creatinine and drugs are added to the tubule. The remaining fluid is urine.

4. Explain that concentration of urine is regulated by counter-current and hormonal mechanisms.

Counter-current: opposite flow in loop of Henle and collecting duct builds a medulla salt gradient; water leaves the collecting duct by osmosis, concentrating urine. Hormonal: ADH (posterior pituitary) increases collecting-duct water permeability when the body is short of water → small concentrated urine; when water is plentiful ADH falls → large dilute urine. Aldosterone increases sodium (and water) reabsorption, supporting blood volume.

5. Explain the causes and treatments of kidney stones.

Causes: concentrated urine, low water intake, excess calcium, oxalate or uric acid, high salt intake, repeated UTIs and some metabolic disorders. Stones are mostly calcium oxalate, sometimes uric acid. Treatment: small stones pass with fluids and pain relief; larger stones are broken by lithotripsy (ESWL shockwaves from outside; PNL through a small incision) or removed surgically; prevention is more water, less salt and oxalate-rich food, and treating the underlying cause.

6. Explain in detail the mechanism and problems related to dialysis.

Mechanism: blood flows on one side of a semi-permeable membrane and dialysate on the other, in opposite directions; wastes (urea, excess salts, water) diffuse down their gradients into dialysate while blood cells and proteins stay. In haemodialysis this happens in a machine; in peritoneal dialysis the peritoneum is the membrane. Problems: infection at access sites, low blood pressure during treatment, tiredness, anaemia, dietary restrictions, time burden, and in peritoneal dialysis, peritonitis (infection of the abdomen).

7. Describe the principles and the problems associated with kidney transplant.

Principle: a healthy donor kidney is surgically joined to the recipient's blood vessels and ureter, taking over filtration, osmoregulation and hormone-related kidney roles. Success needs blood-group and tissue matching. Problems: rejection (immune system attacks the new kidney), so lifelong immunosuppressants are needed; these raise infection and cancer risk and cause side effects (high blood pressure, weight gain, diabetes risk). Donor shortage and surgery risks are further problems.

1۔ Describe the structure of kidney and relate it with its function.

2۔ Explain the detailed structure of nephron.

3۔ Explain the main processes in kidney functioning.

4۔ Explain that concentration of urine is regulated by counter-current and hormonal mechanisms.

5۔ Explain the causes and treatments of kidney stones.

6۔ Explain in detail the mechanism and problems related to dialysis.

7۔ Describe the principles and the problems associated with kidney transplant.

Inquisitive Questions
INQUISITIVE + ANSWERS
1. Hypothesize kidney stone by studying the urine test of relevant patients.

A student hypothesis could be: patients whose urine shows high calcium, oxalate or uric acid, low volume and high concentration are more likely to form stones. The book asks students to study real urine reports, note crystals and pH, and link them to stone type — e.g. calcium oxalate crystals suggest calcium-oxalate stones. Any hypothesis must be checked with a doctor, not used for self-diagnosis.

2. Describe the importance of kidney donation for the benefit of kidney failure patients.

One donated kidney can free a patient from regular dialysis, restore near-normal life, diet and work, and greatly improve survival and quality of life. Because people can live healthily with one kidney, living donation is possible, and deceased donation saves patients who would otherwise wait years for a kidney.

3. Name the important kidney transplant centres in your city.

This is a local-research task: students should list the transplant centres and nephrology hospitals in their own city (for example, in Lahore: Shaikh Zayed Hospital, Jinnah Hospital and Services Hospital have nephrology/transplant services; other cities have their own teaching hospitals). Check the current official hospital list before visiting, as services change.
