Pharmacology is the study of drugs — how they are discovered, how they work, and how they are used safely. Drug discovery begins with target identification — finding the molecule (enzyme, receptor or gene) involved in a disease that a drug could act on. This is the first step, before any compound is tested. Next comes hit identification (screening thousands of compounds for any activity against the target), then lead optimization (chemically refining the best hits for potency, selectivity and safety).
Then the development process begins: preclinical testing in the laboratory and on animals for safety and activity, followed by clinical trials in humans — Phase 1 (small group, safety and dosage), Phase 2 (about 100–300 patients, effectiveness and side effects), Phase 3 (large groups, confirming effectiveness and monitoring rarer side effects), and after approval, Phase 4 (post-market surveillance for long-term effects). Only a tiny fraction of starting compounds survive this years-long pipeline — which is why new drugs are expensive.
Antibiotics kill or stop the growth of bacteria by attacking targets that human cells lack or use differently — this selective toxicity is what makes them safe for us. Penicillin (from the fungus Penicillium) blocks bacterial cell-wall synthesis, so growing bacteria burst; it is highly effective against Gram-positive bacteria. Side effects include allergy (rashes, and rarely anaphylaxis), nausea and diarrhoea.
Tetracycline is derived from soil-dwelling bacteria of the genus Streptomyces; it blocks bacterial protein synthesis by binding the 30S ribosomal subunit — bacteria cannot make the proteins they need, so growth stops. Fluoroquinolones (synthetic) block bacterial DNA replication (DNA gyrase), making them broad-spectrum and bactericidal. Sulfonamides are synthetic compounds derived from sulfanilamide; they block bacterial folic-acid synthesis by competing with PABA. They are bacteriostatic (they stop growth and let the immune system finish the job), selectively toxic because human cells absorb folate from food instead of making it, and are often combined with trimethoprim for a stronger, bactericidal effect. Side effects: rashes, nausea, photosensitivity, kidney damage.
Antiviral drugs treat viral infections by preventing the growth and multiplication of viruses inside the body — for example drugs against hepatitis, influenza and herpes viruses. They usually target a viral enzyme or entry step, which is hard because viruses use the host cell's own machinery.
Antiretroviral drugs are a special group of antivirals designed against retroviruses, including HIV. Retroviruses carry RNA as genetic material and replicate using the enzyme reverse transcriptase (RNA → DNA). Antiretrovirals block reverse transcriptase and other retroviral steps, stopping HIV from proliferating and helping preserve the immune system. Key point from the book: all antiretrovirals are antivirals, but not all antivirals are antiretrovirals — antiretrovirals are the subset aimed at retroviruses.
Monoclonal antibodies are laboratory-produced molecules that enhance the immune system's ability to fight pathogens. Each monoclonal antibody is highly specific to one antigen — that specificity (not broad-spectrum action) is their great advantage: they bind exactly one target and little else, so they act precisely with fewer off-target effects.
Advantages: (1) High specificity — precise targeting of a pathogen, tumour marker or protein. (2) High sensitivity — they can detect tiny amounts of a substance, making them ideal for diagnostic tests (e.g. pregnancy tests, disease screening). (3) Consistency — every batch is identical, unlike antibodies from different animals. (4) Versatility — the same principle serves diagnosis, research (identifying and purifying proteins) and therapy (cancer treatment, autoimmune disease, delivering drugs to tumour cells). (5) Large-scale production from immortal hybridoma cell lines (see Chapter 21) once the line is made.

2. Testing in a larger group about 100–300 patients to test the effectiveness and side effects of drugs indicate __________.
Answer: (b) Phase 2 — Phase 1 is small-scale safety testing; Phase 3 is much larger; Phase 4 is after marketing.
3. Penicillin is highly effective against __________.
Answer: (a) Gram-positive bacteria — their thick peptidoglycan wall is penicillin's target; (d) is the source fungus, not the target.
4. The __________ antibiotic derived from soil-dwelling bacteria of the genus Streptomyces,
Answer: (d) Tetracycline — produced by Streptomyces soil bacteria.
5. The __________ drugs are synthetic compounds derived from sulfanilamide,
Answer: (c) Sulfonamides — synthetic drugs built from sulfanilamide.
6. Monoclonal antibodies are considered highly effective because
Answer: (b) are highly specific to antigens — their precision, not broad action, is their strength; they are biological molecules that bind one target.
Answer: Discovery: target identification → hit identification → lead optimization. Development: preclinical testing (lab and animals) → clinical trials Phase 1 (safety, small group), Phase 2 (~100–300 patients, effectiveness and side effects), Phase 3 (large-scale confirmation) → regulatory approval → Phase 4 post-market monitoring. The whole process takes many years and most candidates fail along the way.
Answer: Mostly mild: nausea, vomiting, diarrhoea, skin rashes. The serious risk is allergy — from mild rash to, rarely, anaphylaxis (breathing difficulty, swelling, shock), which needs emergency treatment. Superinfection (overgrowth of resistant microbes, e.g. fungal thrush) can occur with prolonged use.
Answer: Tetracycline binds the 30S subunit of the bacterial ribosome, blocking the attachment of aminoacyl-tRNA — so the bacterium cannot add amino acids to growing protein chains. Protein synthesis stops and bacterial growth stops (bacteriostatic). Human ribosomes (40S/60S) are affected far less, giving selective toxicity.
Answer: Antivirals treat viral infections generally — they block viral growth/multiplication (e.g. hepatitis, influenza, herpes drugs). Antiretrovirals are a subset of antivirals aimed specifically at retroviruses (RNA viruses using reverse transcriptase), chiefly HIV; they block reverse transcriptase and other retroviral steps. All antiretrovirals are antivirals, but all antivirals are not antiretrovirals.
Answer: High specificity (one target only), high sensitivity (detect tiny amounts — ideal for diagnostics), consistency (identical batches), versatility (diagnosis, research, therapy) and scalability (unlimited production from hybridoma lines). Their precision also means fewer off-target side effects than broad-acting drugs.
Answer: 1) Target identification: find the disease-linked molecule. 2) Hit identification: screen compound libraries for activity against the target. 3) Lead optimization: refine hits for potency, selectivity, stability and safety. 4) Preclinical testing: laboratory and animal studies of safety, toxicity and dosing. 5) Clinical trials: Phase 1 (safety, healthy volunteers/small group), Phase 2 (~100–300 patients, effectiveness and side effects), Phase 3 (large groups, confirm benefit, detect rarer effects). 6) Regulatory review and approval. 7) Phase 4: post-market surveillance for long-term and rare effects. Each stage filters out most candidates, so the process takes a decade or more.
Answer: Penicillin: blocks peptidoglycan cell-wall synthesis → growing bacteria lyse; best vs Gram-positive; side effects mainly allergy. Tetracycline: from Streptomyces; binds 30S ribosome, blocks protein synthesis; bacteriostatic, broad-spectrum. Fluoroquinolones: synthetic; inhibit DNA gyrase/topoisomerase, blocking DNA replication; bactericidal, broad-spectrum. Sulfonamides: synthetic from sulfanilamide; compete with PABA to block folic-acid synthesis (needed for DNA); bacteriostatic, selectively toxic since humans absorb folate from food; often combined with trimethoprim for bactericidal effect. Each class exploits a bacterial target humans lack or use differently.
Answer: Monoclonal antibodies are identical antibodies produced by a single cloned B-cell line (hybridoma technology, Köhler and Milstein, 1975): one antibody type, binding one specific antigen. Advantages: exceptional specificity and sensitivity (precise diagnosis even at tiny concentrations); uniformity between batches; wide uses — diagnostic tests (pregnancy, infections, cancer markers), research tools (protein detection and purification), and targeted therapies (cancer, autoimmune diseases) that hit diseased cells while sparing healthy ones; and unlimited supply from the immortal cell line.
Answer: Challenges: finding a valid target; most compounds fail for toxicity or poor effectiveness; animal results may not predict human results; clinical trials are slow, expensive and need large, diverse samples; resistance can develop (antibiotics); rare side effects appear only after marketing. Solutions: computer-aided drug design and AI screening to find better leads faster; organ-on-chip and better animal models; adaptive trial designs; international collaboration and funding for neglected diseases; strict Phase 4 surveillance; combination therapies and stewardship to slow resistance; repurposing existing approved drugs for new uses.
Answer: Misuse (wrong drug, wrong dose, stopping early, using antibiotics for viral colds) selects resistant bacteria: susceptible germs die, resistant ones survive and spread, so common infections become untreatable and routine surgery becomes risky. Overuse also damages the normal gut flora (diarrhoea, fungal overgrowth), causes side effects and allergies, and raises healthcare costs. Consequences are already visible in multidrug-resistant tuberculosis, MRSA and resistant urinary infections. Prevention: prescribe only when bacterial infection is confirmed, complete the full course, never share antibiotics, and reserve last-line antibiotics for when they are truly needed.
